WALFORD, M.D. T HE CLINICAL ENTITY of syphilitic paroxysmal cold hemoglohimsuria has beens adequately described by Rosensbach,’ Domsath amid Landsteiner,2 Mackenszie,3 Ross4 amid others. Ins this disease exposure to cold induces a paroxysm characterized by a shakinig chill, fever, flank pain amid hemoglobinuria. This phensomensoni is thought to be due to a cold hemolysims found ins the patienit’s serum. Ins vitro experiments have showms that this antibody will sensitize the patienit’s erythrocytes, or those of another insdividual, upon exposure to cold. Subsequenst inscubations at 37 C. ins the presence of complement will bring about lysis of the senisitized cells. The most recemst amid detailed work oni theexact meehamsism of this reactions (the Donsath-Lansdsteiner reactions) has heens done 1w Jordan. Pillemer and Dingle.” 6

Syphilitic paroxysmal cold haemoglobinuria. Case report and study of the Coombs test.

A case of congenital syphilis which presented with paroxysmal cold haemoglobinuria is described. The patient had aortic incompetence, and the significance of this finding is discussed. The Coombs test was studied, and positive results appear to be due to interaction between antiglobulin serum and complement adsorbed on to the test cells, this adsorption being promoted by the cold antibodies.
